1. Completely fill out the online dietary history form here (this link will take you to free online tool at BalanceIT.com ; please enter your Vet's Email address as info@vnss.vet ).

Please refer to "Online Diet History Form Instructions" to learn how to correctly fill out the diet history.

2. Please have your veterinarian email or fax +1-415-457-6318 all of the medical records* including recent lab tests and diagnostic imaging results.

*For healthy pets: A full physical examination by a California-licensed veterinarian within 1 year of scheduling an appointment is required. For sick pets: An established diagnosis or recent (within 3 months) lab work including serum chemistry, complete blood count, +/- urinalysis is required.

Remote consultations are available for patients residing in California only

Please email or call +1-415-375-0514 (for veterinarians only*) to speak to Dr. Yuki Okada about the case specifics and current turnaround timing given caseload. *Calls to this vet-only number by clients will be immediately redirected to call +1-415-456-7372 to set up an in-person appointment as phone consultations with clients are unfortunately not currently available.

2. Any needed/supporting medical records for remote consultations should be emailed to info@vnss.vet .

3. Please instruct your client to completely fill out the online dietary history form here and enter the veterinarian's email address as info@vnss.vet (not yours), so we automatically receive a copy which we will reference as needed.
